The first thing you should know is that the level of the cows can be visited in Normal, Nightmare, or Hell difficulty and that it is required to have completed the game in one of these to be able to access this secret.
Once you start a new game on the difficulty in which you have completed the game, you will have to get hold of a series of unique objects to be able to unlock the portal that will lead us to the level of the cows.
Tome of the city portal: it is the easiest item to get. You will be able to get hold of it in the markets of the different towns.
Wirt's leg: you will have to go to Tristan from the Stony Field portal and look for Wirt's corpse. If you examine it you can get this item.
Horadric Cube: this one does not have much mystery, since by completing the second mission of Act 2, you will be able to get it.
Once the ingredients are gathered, the recipe needs to be finished. To do this, you must go to the Harpies Camp, and do the following: Go to the inventory and select the Cube. Insert Wirt's leg and tome inside and perform a transmutation of these objects.
